1. The Gift Certificate includes the cost of the Thimble Slide plus shipping (if any) and will show your name as the Giver (fill in the "Contact Information" fields with your info) and the person's name you're giving it to (in the Summary section, fill in the "Notes Or Instructions" field with their name).
